If you have won $600 or more, you will need to complete and sign a Winner Claim Form.

Any prize can be claimed in person at the Phoenix or Tucson office or by mail. To claim your prize by mail, send the signed ticket, completed Winner Claim Form, and a copy of a governmental identification that includes your mailing address and signature (such as a driver's license, citizenship/residency card) to:

                   Box 2913
                   Phoenix, AZ 85062-2913

Be sure to keep a copy for yourself.
